Open Letter to Individual Car Owners and Independent Repair Facilities

CARE needs your help in documenting incidents where the Automotive Aftermarket is unable to fix/repair a customer's vehicle due to a lack of information and/or diagnostic tools. CARE has developed two forms -- : one for Individual Car Owners, and a second for Independent Repair Facilities -- to use in documenting each such case. Click on the appropriate form name to: (1) view the document; (2) complete the document; either online or by printing out a hard copy, and (3) faxing, mailing, or e-mailing the completed form to CARE.

There is no pride of authorship attached to these forms and you are welcome to modify them as you see fit. Also, feel free to print copies of these forms for distribution to others who may be interested in helping.

Your assistance will be invaluable in building a large database of irrefutable evidence that vehicle owners are able to have their vehicle repaired in the Automotive Aftermarket if that is their choice. The completed form, in whatever format you use, needs to be returned directly to CARE. CARE will enter all information into a database. If necessary, CARE will call the individual/repair facility to clarify the information submitted. the information gathered through this process will be used to directly support H.R. 2735.
